As the parent and/or guardian of above named child, I authorize my child’s participation in the Maritime Spirit
Cheerleading (MSBC, Inc.) classes, and agree to accept full financial responsibility for all fees, costs, damages, and
expenses incurred or caused by the participant in all aspects of the program and/or use of the facilities.

I further authorize MSBC, Inc and Clarkbridge Hall or their agents to exercise their judgment to seek and obtain medical
care for the participant in the event of an acute illness or injury necessitating such care.  I, as parent and/or guardian
agree to pay all expenses incident to such illness or injury; including, but not limited to; transportation, evaluation,
diagnostic testing, and treatment.  I hereby release MSBC, Inc., Clarkbridge Hall, their respective board members,
members, volunteers, staff and agents from all liability for injury or loss of property for the above listed participant.

I agree that MSBC, Inc. and Clarkbridge Hall along with their respective officers, staff, and agents assume no
responsibility for accidents or injuries or for any loss or damage of personal property.  MSBC, Inc. reserves the right to
combine or cancel classes as deemed necessary due to unforeseen circumstances. Refunds only given upon MSBC Inc.’
s cancellation or participant having a dated doctor’s confirmation deeming them unable to participate for rest of session.  
Every effort will be made to make up classes cancelled due to inclement weather.
Parent waiver signature:
Payment may be made the first class session.
Preschool Cheerleading Class

For youth ages 4-5.  This class will teach basic cheerleading movements,
jumps, and cheers.  Participants will demonstrate skills learned at Kid’s Expo
in Manitowoc on Saturday, March 27th, 2010.
 
Wednesdays: February 3rd through March 24th (8 weeks)
3:30 pm to 4:00 pm
Fee: $28.00

Participants should dress comfortably in athletic wear, no jewelry, long hair pulled back.   
Tennis shoes should be worn during class.

Classes are held in our facility located at 1402 Clark Street in Manitowoc.  Please use
front parking lot entrance off of Clark Street.
